Activity Log
Part of the documentation process that you will need to do for your program is to keep an Activity Log. This is simply a tally of the various tasks that you took part in, with details such as the date, a brief description of the work done, and the respective supervisor's signature.
Download the FORM and fill in the needed information. Then make sure to have your respective supervisors sign off on the sheet each time you do non-trivial work for your activity. It would be best to have separate Activity Logs for each long term project or activity. Short term or one-off activities (i.e. Coastal Cleanup, Food Festival, etc.) can be combined into one Log sheet. This will serve as record of attendance and can be scanned/imaged then posted on the blog at the end of the project or activity.
For activities that meet daily (like varsity sports, for example) you can make arrangements with your supervisor to get signatures on your logs on a weekly basis instead of daily. For clubs that meet once or twice a week, supervisors must sign the logs every meeting.
*It is the student's responsibility to have the Activity Log available for supervisor signatures.
**It is expected that multiple copies of this log sheet will be used over course of a student's CAS program. Please print on recycled paper whenever possible.
